Renew your Passport in three easy steps

Step 1
Print each document that you will need. Then complete each form using a black pen. When you have completed all forms call us at 1-409-385-2438, and we will go over the process with you.
Step 2
Take all completed forms and necessary documents to your local Acceptance Agent. The Acceptance Agent will give you a sealed envelope (do not open it). Click here to find the Acceptance Agent nearest you.
Step 3
Fedex (overnight) the sealed envelope along with the National Passports Order form and payment to us at National Passports 335 HWY 327 West  #286 Silsbee, Texas 77656
Renewal
  • you have your expired passport or your passport is going
    to expire in the next two years.
  • your passport was issued less than 15 years ago.
  • your passport was issued after you turned 16 years old.
  • your name is the same as it is on the passport or if not, you have proof of your name change.
You will Need the following items to obtain a second passport...
DS-0082 Red Ammendment Form
  Expired Passport
  Authorization Letter Child (If applicable)
  Authorization Letter Adult (If applicable)
  Proof of Departure (if traveling within 10 days)
  2 Passport Photos
  Request for a Second passport
  Check or Money Order: Payable to Department of State. Adult $135.00, Child $120.00
