New Passport in three easy steps
Step 1	Print each document that you will need. Then complete each form using a black pen. When you have completed all forms call us at 1-409-385-2438, and we will go over the process with you.
Step 2	Take all completed forms and necessary documents to your local Acceptance Agent. The Acceptance Agent will give you a sealed envelope (do not open it). Click here to find the Acceptance Agent nearest you.
Step 3	Fedex (overnight) the sealed envelope along with the National Passports Order form and payment to us at National Passports 335 HWY 327 West #286 Silsbee, Texas 77656

New	you have never had a passport. your passport was issued more than 15 years ago. your passport was issued before you turned 16 years old. you are applying for a minor under the age of 16 and you are their legal guardian.
You will Need the following items to obtain a NEW passport...
	DS-11 Brown Passport Application (sign in front of Accepting Agent)
	Certified Birth Certificate (Click here if you need us to obtain your Birth Certificate)
	Authorization Letter Child (If Child Applicant)
	Authorization Letter Adult (If Adult Applicant)
	Statement of Consent (needed if child is leaving the country with only one parent)
	Proof of Departure (if traveling within 10 days)
	2 Passport Photos
	Drivers License w/Picture (if Child Applicant -Parents Drivers License w/Picture. Drivers license must have been issued more than one year ago. If the last name difffers from the childs, the law requires legal documents for proof of parentage.) enlarge copy by 200% and lighten
	Check or Money Order: Payable to Department of State. Adult $135.00 Child $120.00
	Check or Money Order: $30.00 Payable to the Acceptance Agent.
